POSITION
Summary:
Investigates, assesses and plans interventions to help patients cope with social, emotional, economic and environmental problems.
PRINCIPAL ACCOUNTABILITIES:
1.Essential Job Functions:
a. Interviews patients, assesses priorities and documents case activity.
b. Consults with team managers, statutory and voluntary agencies and patient's relatives to ensure compliance with patient's treatment plan.
c. Assists clients/families with obtaining community assistance by referral to proper resources.
d. Develops and maintains working relationships with community health, welfare and social agencies and seeks creative means to assist the client with his/her needs.
e. Counsels clients in individual and group sessions to assist in dealing with substance abuse, mental and physical illness, poverty, unemployment, or physical abuse.
f. Collaborates with counselors, physicians, and nurses to plan and coordinate treatment, drawing on social work experience to meet patient needs.
g. Monitor, evaluate, and record client progress with respect to treatment goals.
h. Counsels and aids family members to assist them in understanding, dealing with, and supporting the client.
MINIMUM
Qualifications:
Bachelors degree required. MSW degree from an accredited social work program required.
Five years' experience working with substance abuse population required.
Prior experience in acute care hospital social work, psychiatry, and/or mental health preferred.Estimated Salary: $20 to $28 per hour based on qualifications.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
