Statement of Purpose/Summary of Major
Essential Functions:Coordinates, directs and facilitates the integration of Social Services functions into the resident care, and discharge planning processes with the Interdisciplinary Team (IDT) ensuring compliance with Federal andDistrict of Columbiaregulations.
Education and Experience Requirements:Accredited college or university.
Minimum
Experience:Two years experience in a health care setting (preferably Nursing Facility). Excellent written and verbal skills. Knowledge of and ability to effectively use community resources, ability to plan, organize, and set priorities.
Licensure/Certification Requirements:BSW or MSW from an accredited college or university, current DC Social Worker License or verifiable application for one.Must maintain current provider CPR certification throughout employmentEstimated Salary: $20 to $28 per hour based on qualifications.
