Our caregivers focus on each unique person to deliver the appropriate care and emotional support to our clients and their families.
Job Responsibility's:
oBe primarily responsible for internal auditing, RN, LPN and HHA clinical record reviews and OASIS submission reviews to ensure compliance with federal and District of Columbia regulatory requirements.
oProactively seek opportunities to assist with the identification of risks to patient safety.
oEstablish necessary action plans in collaboration with the management team to improve documentation and patient safety.
oProvide staff education regarding quality assurance and process improvement activities for the Home Health Care documentation.
oWork closely with all departments to achieve quality outcomes and assure competency of staff.
oConduct active and discharge clinical record reviews with follow through to appropriate supervisor/manager to improve quality and adherence to regulatory requirements.
oEnsure adherence to CMS Conditions of Participation, Medicare, and Medicaid guidelines for home health services.
Education /
Experience:
oMust possess thorough knowledge of home health practices, procedures, and professional standards of care as set by Federal and state regulations, as well as Medicare regulations for home health services.
oMinimum two (2) years of home health care or long-term care experience; previous acute care experience preferred.
oPrevious Quality Assurance experience and/or Performance Improvement experience.
oStrong clinical judgement and critical thinking skills; strong verbal and written communication skills.
oPrevious experience with Electronic Health Records(EHR)/ Electronical Medical Record (EMR) systems (AXXESS preferred but not required).
oFamiliarity with Oasis and ICD 10.
oRN license in good standing in the District of Colombia Hours:
