Registered Nurse Case Manager- Palliative & Supportive Care Program:
In addition to responsibilities related to the day-to-day operations of the palliative care program, this position serves as a palliative care resource to patients, families, members of the health care team.
Clinical work is performed with general independence and decision-making responsibility in accordance with palliative care clinical practice guidelines and scope of practice, and the attending palliative care provider.
Guidelines include general policy procedures and practice guidelines established by the department and UWMC, Northwest Campus, as well as appropriate professional and technical manuals.
In conjunction with the attending palliative care provider, this individual shares responsibility for providing palliative care clinical education to physicians, nurses, social workers, chaplains, and others as requested.
This individual assists with the coordination and placement of palliative care patients in the inpatient and outpatient settings.
Essential Functions:
Palliative Care:
Is knowledgeable about the plan of care for patients, followed by attending palliative care providers.
Serves as a triage nurse for ALL incoming calls.
Serves as a resource for palliative care patients across all settings.
Provides education to patients, families, medical and nursing staff regarding hospice care as requested.
Assists, resolves, and/or appropriately refers any customer service issues to appropriate resources.
Assists the manager and Campus Palliative Care Section Head with orientation and education of medical and surgical residents regarding palliative care issues, both formally and informally.
Works with attending palliative care providers to ensure that a palliative plan of care is communicated to members of the healthcare team as well as to patients and their families.
Consults with and supports social workers, therapists, care coordinators, and charge nurses throughout the NW Campus in response to questions/issues.
Provides information and direction to patients and their families about support groups.
Maintains the palliative care service patient database following national standards.
Compiles service data for analysis.
Coordinates palliative care teaching rounds as requested.
Performs various administrative tasks to support departmental operations and programs.
Examples include but are not limited to:
oCoordinates billing/patient charges, arrange payments and processes reimbursements oAssists with budget preparations and tracking expenses oCompiles workload statistics and metrics.
oAssists with updating procedures and department forms, etc Assists the manager and Campus Section Head as requested with the development of policies and guidelines for the provision of palliative care across all settings.
Contributes to the professional development of peers, colleagues and others, including members of the interdisciplinary team.
Assists with ensuring a coordinated and aligned system-approach through regular communication and attendance at system-level meetings as appropriate.
Serves as a resource for attending and resident physicians, medical students, graduate and undergraduate students, and other members of NW Campus staff.
Collaboration and Communication Works collaboratively with social work, spiritual care, and other disciplines in the provision of palliative care.
Serves as an effective resource for attending and resident physicians, medical students, graduate and undergraduate students, and other staff members Orients new employees and assists other team members with skill development regarding hospice/palliative care.
Provides constructive feedback and supports team-based care.
Effectively communicates with other staff members and management to assure continuous service to patients and families.
Supports patient satisfaction and customer service initiatives, providing timely resolution from any customer service issues.
Participates in interdisciplinary teams, committees, projects, and work groups.
Effective interpersonal skills, leadership skills, and experience in the care of patients with a life-limiting illness.
Special Training / Skills Time management, multi-tasking, and effective prioritization/organization skills.
Demonstrated ability to effectively deal with all levels of staff, physicians, patients and visitors/families, both internally and externally, using a professional and courteous manner.
REQUIREMENTS:
Education Background:
Graduate of an accredited school of nursing.
Licensure, Registration, Certification Registered Nurse with current Washington State license.
Obtains certification as a hospice and palliative care nurse within one year of hire.
Work Experience Minimum of two years of professional nursing experience working with people at the end of life.
Minimum of two years' experience in a management, supervisory, or charge nurse role.
Special Training/Skills Effective interpersonal skills, leadership skills, and clinical expertise in the care of patients with a life-limiting illness.
Skills Ability to demonstrate the knowledge and skills necessary to provide care appropriate to the age of patients served on the assigned unit/department.
Ability to flex to assist other department members as needed.
Psychiatric assessment skills at a level consistent with social work job assignment Ability to handle a fluctuating workload Ability to communicate clearly in English (verbal and written) with all stakeholders (staff members, clinicians, practitioners and the general public.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
