The Harborview Adult Medicine Clinic has an outstanding opportunity for a Medication Assisted Treatment (MAT) Program Coordinator (Full-Time, Day Shifts).
Under general supervision, this position coordinates the delivery of medical care, mental health, recovery and other social services for patients requiring medication-assisted treatment for opioid use disorder and access to health care.
The Program Coordinator facilitates and tracks the patients' entry into and through the spectrum of clinical services of the health care system, spoke clinics as well as makes referrals to a spectrum of other services.
The Program Coordinator requires specialized training and understanding of the complex patient groups.
This position is also responsible for coordinating outreach AB2 and engagement with the peer navigators, the nurse care manager and other multidisciplinary team members, to provide patient care.
Duties include:
Program Outreach:
Conduct outreach through email to HMC clinics, providers to offer linkage services to patients identified as targeted client base.
Coordinate with personnel from Public Health - Seattle & King County, other treatment programs and area clinic managers and medical providers to provide information on MAT and related services and to coordinate with regard to clients identified as needing assistance with MAT.
Coordinate with case managers and inpatient medical providers to assess need for linkage to MAT care among persons hospitalized in HMC or in aftercare.
Collaborate with providers and staff of out-patient clinics to inform them about MAT and coordination of various health, social service and recovery activities Develop and maintain procedural standards for outreach efforts, including what requires data entry and what other care team members must do.
Attend meetings as the spokesperson for MAT/outreach efforts Provide insight and follow-through on improvements to the outreach processes, including its interface with others (healthcare clinics and in-patient hospital staff, contact center staff, providers, etc.
) Coordinate with outside resources for care gap follow-up and decreasing barriers to patients obtaining screening.
Care Coordination:
Handle intake of all referral requests, ensuring complete information is received and entered into the computer system.
Provide telephone screening of patient medical problems and severity of condition.
Triage patient telephone calls to clinical staff; initiate emergency procedure when necessary.
Coordinate incoming and outgoing referrals, Provide information and advice regarding treatment services to prospective patients and their advocates, to personnel from outside agencies, staff from outside hospitals, and the general public in addition to the patient population.
Act as primary contact for patients being referred to MAT services.
Provide messaging and instructions to patients about MAT, about screening, and treatment.
Implement a screening questionnaire to each prospective patient to assess eligibility and fit for the program.
Interview patients and review anticipated needs upon admission to the program and discharge, including:
personal escort, transportation, housing, support care, equipment needs, transfer of records, and appointments with physicians and care managers.
Ensure complete and accurate registration of patients to receive MAT services, including patient demographic and insurance referral information are in the computer system.
Ensure a complete medical record is present at the time of patient visit, admission, and/or procedure.
Manage Epic referral queues and other databases.
Educate clients as needed about related recovery programs and may inform patient about mental health services, social services and other programs or services related to their recovery.
Coordinate referrals and follow-up activities as needed to link clients with recovery-related support, mental health, medical and financial and health care coverage services.
Appropriately direct patients and others to other departments across the medical center.
Schedule various appointments for patients.
Under general direction and using knowledge of services, independently advise patients and prospective patients on accessing health care.
Have extensive involvement with new and returning patients, case managers, nursing staff, physicians and other clinic staff in carrying out program activities.
Coordinate clerical patient care activities and/or specialized technical duties unique to the assigned clinic to assure accurate and expedited patient check-in, including obtaining consents for care and referral information.
Assist with completion of patient forms, applications, and other paperwork.
Complete appropriate documentation for referral to clinic programs.
Facilitate the flow of documents between patient, providers and payers.
Facilitate sending of treatment summaries to patient's primary care physician.
Oversee health care efforts through assistance managing patient registries and provider panels and patient outreach processes in coordination with attendant HMC clinics and outside resources.
Review appointment follow-up requirements and expectations with patient to ensure adequate and timely review of the patient plan of care in accordance with all CMS and other regulatory requirements.
Act as resource to the team for payor issues including insurance knowledge for Medicare, Medicaid and commercial plans.
Facilitate prior authorization with insurers for payment of treatment.
Coordinate with the case managers, financial counseling and the Contact Center for verification of benefits and authorization for outpatient visits.
Refer patients to Financial Counselors when needed.
Ensure efficiency and continuity of efforts across quality improvement/health care efforts and recommends and initiates changes as needed; must have telephone conversational and screening skills to assist in the reconciliation of registries/screening data.
Participate in patient care coordination, reports and records observations/updates regarding needs.
Record and manage data related to program activities and goals; respond to quarterly reporting requirements.
Maintain strict confidentiality of medical and program records.
Provide summary reports to clinic leadership on referral volumes and status.
Prepare MAT patient care reports for care team meetings and assists in resource development and tracking.
Work with clinic care teams in supporting clinical activities related to recovery, mental health, chronic care and other health care services (running reports, identifying care gaps, calling patients, making appointments, obtaining screening results, entering data into the medical record.
Communicate with case managers and health care team members regarding the status and/or completion of care coordination activities.
Participate in the orientation of new staff to MAT patient care efforts.
Contribute to the development of policies and procedures.
Participate in staff meetings and care management/coordination meetings.
Office Tasks:
Process clinic charge documents to assure completion and accuracy within an acceptable time frame.
Perform office tasks as needed including distribution of mail, creation of clinic-specific patient packets, and processing loose medical records papers.
Maintain and use supplies and equipment in a cost-effective manner.
Respond to inquiries by the appropriate route (telephone, fax, or email) and route to the appropriate individual.
Provide support in data management and tracking as needed, and consistently meet assigned timelines.
Demonstrate excellent time management skills by prioritizing appropriately when busy, and effectively utilizing downtime in productive manner.
Proactively bring suggestions to manager/supervisor.
Participate in positive manner on clinic/ divisional process improvement initiatives.
Other Duties:
Participate in orientation of new staff, including providers/ residents, as requested.
Cross-train to cover areas beyond main assignment.
May provide back-up for Business Operation Supervisor on limited/temporary basis.
Collaborate effectively with other departments.
REQUIREMENTS:
High school graduation or equivalent AND two years of experience in the program specialty OR equivalent education/experience.

Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
