Resource Coordinator - Cancer Center

Position SummaryThe Resource Coordinator under the direction of GW Cancer Center will navigate patients throughout the cancer care continuum to address health care disparities and barriers to care in order to improve the quality of life for patients and families.
The primary goal of the Resource Coordinator is to decrease fragmented care, coordinate services, and assist patients in accessing cancer care and navigating healthcare systems.
The Resource Coordinator specifically aims to guide patients through the collaborative process with all care team members by addressing non-clinical barriers to care and connecting patients with existing cancer support resources.
Essential Duties and Responsibilities to include the following.
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

Conducts initial and ongoing comprehensive needs assessments of patients newly diagnosed with cancer and/or established patients receiving treatment.
Follow up with patients to support adherence to an agreed-upon treatment plan through continued collaboration with the clinical team.
Initiates and encourage active communication with patients and families between physicians, ancillary services, and other disciplines to optimize patient outcomes.
Collaborates with representatives of community programs to educate patients on local programs and facilitate referral of patients to those sources of assistance.
Identify and assist patients facing financial hardship with enrollment into patient assistance programs including the Katzen Patient Assistance Fund.
Maintain comprehensive, timely, and legible health records capturing ongoing patient barriers, patient interactions, and barrier resolution.
Use knowledge of one's role and the roles of other health professionals to appropriately address and optimize safe and effective patient and population centered careAdvocate for quality patient care and optimal patient care systems to advance health equity, improve quality, and reduce health disparities.

EducationBachelor's degree in a related field or equivalent knowledge.
OPN-CG (Oncology Patient Navigator-Certified Generalist) preferred or obtained within 12 months of hire.
ExperienceThis position requires a minimum of 2 years of healthcare experience, preferably in oncology.

Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
